Acne Scars

Acne scars on the face, chest, and back are very common. Around 80% of people between the ages of 11 and 30 experience acne, and about one in five of them develop scars. To reduce these scars, treatment is needed, which may include over-the-counter medicines or procedures done by a dermatologist.

What Causes Acne?

Acne happens when pores (tiny openings in the skin) get clogged with oil, dead skin cells, and bacteria. Normally, thousands of skin cells shed every hour, but sometimes they block pores. This can lead to small clogs like whiteheads or blackheads, or more inflamed pimples.

Acne is very common and affects about 80% of people between ages 11 and 30. For teenagers, it usually appears due to changing hormones. Adults can also get acne because of stress, hormonal changes, menstrual cycles, oil-based products, environmental factors, or even birth control pills.

Types of Acne and What They Look Like

Acne can appear in several different forms, ranging from mild to severe. While whiteheads and blackheads are the most common and usually heal without leaving scars, other types can be more painful and often increase the risk of scarring:

  • Papules: Small, pink to red bumps that are often tender or painful when touched.

  • Pustules: Pimples filled with pus. They appear red at the base with a white or yellow tip.

  • Nodules: Large, firm, and painful lumps that form deeper under the skin compared to papules or pustules.

  • Cysts: The most severe type of acne. These are deep, pus-filled, painful lesions under the skin and are most likely to cause permanent scars.

👉 Recognizing the type of acne you have is important, as the right treatment depends on its severity.

How Does Acne Cause Scars?

Acne scars form when breakouts damage the skin and the body tries to repair it. Normally, pores affected by acne swell due to inflammation. In mild cases, the damage is small, and the skin heals quickly with little to no scarring. But when the contents of a breakout (oil, bacteria, and dead skin) spread deeper into the skin, it damages surrounding tissue and leaves behind more noticeable scars.

The healing process involves the production of collagen (the protein that repairs skin). If too little collagen is produced, it leaves behind depressed or indented scars. If too much collagen is produced, it creates raised scars.

It’s important to note that not everyone with acne will develop scars — only about 1 in 5 people with acne experience scarring. And even if scars do form, they are not always permanent. Many effective treatments are available today that can reduce or even almost remove acne scars, while others help the skin naturally rebuild with its own collagen.

What Are the Types of Acne Scars and What Do They Look Like?

Acne scars don’t all look the same. In fact, most people with acne scarring have a combination of different scar types, rather than just one. Each type has its own appearance and depth, and they respond differently to treatments.

1. Atrophic (Depressed) Scars

These scars form when the skin doesn’t produce enough collagen during healing, leaving indents.

  • Ice Pick Scars: Narrow, deep holes in the skin, wider at the surface and tapering to a point—much like a puncture from an ice pick. These are common on the forehead and upper cheeks where skin is thinner.

  • Rolling Scars: Indented scars with smooth, sloping edges that make the skin surface look uneven or wavy. They usually appear on the lower cheeks and jawline.

  • Boxcar Scars: Broad depressions with sharply defined edges. They go deeper into the skin than rolling scars and are common on the lower cheeks and jaw.

2. Hypertrophic and Keloid Scars

These scars form when the skin produces too much collagen during the healing process. Instead of being depressed, they rise above the skin’s surface.

  • Hypertrophic Scars: Thick, raised scars that stay within the boundary of the original acne spot.

  • Keloid Scars: Larger, raised scars that extend beyond the original acne area. They often occur on the chest, back, shoulders, and jawline. Keloids can be itchy, tender, or even painful.

What in-office treatments work best for acne scars?

Dermatologists have several cosmetic procedures that can improve acne scars. Often, more than one treatment or multiple sessions may be needed to get the best results.

Resurfacing Procedures

These treatments remove damaged skin layers and encourage new, smoother skin growth.

  • Chemical Peels: Special solutions remove the top layer of skin, letting new, smoother skin grow.

  • Dermabrasion: A tool gently “sands” the skin, removing the upper layers.

  • Microdermabrasion: A milder version of dermabrasion that uses a special machine.

  • Laser Resurfacing: Heat from a laser stimulates collagen production and helps skin heal with fewer scars. (Two types: ablative and non-ablative.)

Other Procedures

  • Steroid Injections: Help flatten raised scars by softening excess tissue.

  • Dermal Fillers: Fill in depressed scars, lifting them up to skin level.

  • Microneedling: Tiny needles stimulate collagen production for smoother skin. (Can also be combined with radiofrequency for stronger results.)

  • Excision: The scar is surgically removed and the skin is stitched closed.

  • Subcision: A needle loosens the fibrous bands pulling scars downward.

  • Punch Grafting: The scar is cut out and replaced with a skin graft from another body area.

  • Cryosurgery: Liquid nitrogen freezes raised scars, causing them to shrink and fall off.

👉 The right option depends on your scar type, skin condition, and healing capacity, so it’s best to discuss with your dermatologist.

How Can I Reduce My Risk of Acne Scars?

  • Always wear sunscreen: Sun exposure can make scars darker and more visible, especially on darker skin tones. Protect your skin daily with sunblock.

  • Don’t pick or squeeze pimples: Picking, scratching, or squeezing can worsen inflammation and leave permanent scars.

  • Treat acne early: The sooner you manage breakouts, the lower your chances of developing scars.

  • Avoid smoking: Smoking slows healing and increases the risk of scarring on both the face and body.

  • Consult a dermatologist: Professional care ensures timely treatment and reduces the risk of long-lasting marks.

Frequently Asked Questions

Not always. Mild acne like whiteheads and blackheads usually heal without scars. Inflammatory acne such as nodules and cysts are more likely to leave marks.

Shallow scars and post-inflammatory marks may fade over time, but deeper scars usually require treatments like microneedling, chemical peels, or laser therapy.

Dark spots are pigmentation changes (post-inflammatory hyperpigmentation) and usually fade with time or treatment, while acne scars are changes in the skin’s texture.

You can begin treatment once your acne is under control. Treating scars while still having active breakouts may worsen irritation.

Natural remedies like aloe vera or honey may soothe skin but won’t significantly improve deep scars. Professional treatments give better results.

Treated scars won’t return, but if acne flares up again and isn’t managed properly, new scars can form.

Yes, but the type of laser used depends on your skin tone and scar type. A dermatologist will recommend the safest and most effective option.

Results vary. Some procedures like chemical peels show improvement within weeks, while others like microneedling or laser may need multiple sessions over months.
