Butt Lift Surgery – Lift and Shape Your Buttocks

A buttocks lift removes excess skin and tightens the area for a firmer, more toned look.

Butt (Buttocks) Lift

A butt lift is a cosmetic surgery that removes extra fat and loose skin from the buttocks to make them look firmer and more lifted. It’s a major procedure with a longer recovery, but if you maintain a stable weight, the results can last a long time.

Sometimes, a butt lift is done along with a tummy tuck or a lower body lift to improve the shape of the buttocks, thighs, hips, and abdomen. During the surgery, the extra skin and fat are removed, and the remaining skin is tightened for a more toned look.

What is a Butt Lift?

A butt lift, also called a gluteal or lower back lift, is a cosmetic surgery that tightens and lifts the skin around the buttocks. This improves the shape, smoothness, and firmness of your butt, creating a more toned look. Unlike some other procedures, a butt lift does not make your butt bigger.

Who is a Good Candidate?

A butt lift may be right for you if you:

  • Are healthy and don’t smoke.

  • Don’t have obesity.

  • Don’t have chronic health issues like diabetes or lupus.

  • Have lost a lot of weight or had weight loss surgery.

  • Have extra fat, sagging, or loose skin on your buttocks.

Who Should Avoid a Butt Lift?

You may not be a good candidate if you:

  • Can’t commit to a longer recovery period.

  • Can’t stop smoking at least six weeks before surgery.

  • Have serious health problems like heart disease.

  • Have unrealistic expectations about results.

  • Haven’t reached a stable weight.

Butt Lift vs. Brazilian Butt Lift (BBL)

While the names sound similar, the two procedures are very different:

Butt Lift – Lifts and tightens sagging skin for a smoother, toned appearance.
BBL (Brazilian Butt Lift) – Adds volume to the butt by transferring fat from another area of the body.

BBL Procedure Steps:

  1. Fat is removed from areas like your stomach or thighs using liposuction.

  2. The fat is purified and injected into your buttocks to increase size and shape.

Why a Butt Lift is Done

As we age, our skin naturally loses elasticity and becomes looser. Factors like sun damage, weight changes, and genetics can make it harder for skin to bounce back, leading to sagging in the buttocks and other areas.

A butt lift is often done along with other body contouring surgeries to improve overall shape. You might consider a butt lift if you:

  • Have lost a lot of weight and kept it stable for at least 6–12 months.

  • Are overweight and unable to lose a significant amount of weight through exercise and diet.

  • Are at a healthy weight but want a more sculpted lower body.

  • Have a healthy weight but are left with loose skin after liposuction.

Note: A butt lift won’t improve your skin’s natural texture or quality—it only reshapes and tightens it.

When a Butt Lift May Not Be Right for You

Your doctor may advise against the procedure if you:

  • Have serious health conditions like heart disease or diabetes.

  • Plan to lose a lot more weight.

  • Have a BMI over 32.

  • Smoke or are unable to quit before surgery.

  • Have an unstable mental health condition.

How to Prepare

Before surgery, you’ll meet with a plastic surgeon who will:

  1. Review your medical history – including past and current health conditions, medications, and surgeries.

  2. Discuss weight changes – especially if your interest in the surgery is linked to weight loss.

  3. Do a physical exam – to check your buttocks, skin, and lower body. Pictures and blood tests may also be done.

  4. Talk about expectations – to ensure you understand the results, scarring, and possible risks.

You may also need to:

  • Stop smoking before and after surgery to help healing and reduce complications.

  • Avoid certain medications like blood thinners, aspirin, and herbal supplements that increase bleeding risk.

  • Maintain a stable weight for 6–12 months before surgery.

  • Arrange help for after the procedure, including a ride home and assistance during recovery.

What to Expect

Before Surgery:

  • The procedure is done in a hospital or surgical center.

  • You’ll be under general anesthesia (completely asleep).

  • A bladder tube may be placed, and your legs will have compression boots to prevent blood clots.

During Surgery:

  • An incision is made across the lower back, from hip to hip.

  • Skin is lifted, excess fat and skin are removed, and the remaining skin is pulled up to lift the buttocks.

  • Antibiotics are given to prevent infection.

  • The surgery takes 2–3 hours (longer if combined with other procedures).

After Surgery:

  • The incision will be covered with dressings, and drains may be placed to remove excess fluid or blood.

  • You’ll be encouraged to walk the first day to help prevent blood clots.

  • Pain will be managed with medication.

  • Drains might remain for several weeks, and you’ll be shown how to care for them.

  • You may need antibiotics and blood clot prevention medicine.

  • You’ll wear a supportive garment for several weeks to help healing and reduce swelling.

  • Scar care may include silicone sheets, creams, and massage.

  • You’ll need to move carefully for the first few months and avoid positions that put strain on your incision.

  • Regular follow-up visits will be necessary.

Results

A buttock lift can make your buttocks look more toned and firm. The results usually last for many years, but maintaining a stable weight is key to keeping your new shape.

Risks

A buttock lift comes with possible risks, including:

  • Fluid buildup (seroma): Fluid may collect under the skin. Drainage tubes after surgery can help prevent this, or your surgeon may remove the fluid with a needle.

  • Poor wound healing: Some areas along the incision might heal slowly or open up. Antibiotics may be prescribed if this happens.

  • Scarring: Scars are permanent but usually placed in areas that are not easily visible.

  • Changes in skin sensation: Numbness or reduced feeling in the area is common and often improves in a few months.

  • General surgery risks: Bleeding, infection, and anesthesia-related complications.

If you combine a buttock lift with augmentation using your own fat, there are additional risks like infection and, in rare cases, death.
